    *We accept clients claiming on their OON benefits and provide superbills that can be filed with the insurance company. 

    EAP- Employee Assistance Program

    EAPs will not be accepted for HRT letters and will only be accepted ONCE per calendar year per client/couple unless a dire clinical need is present. Clients will be expected to use Insurance and/or self pay once initial EAP sessions are complete.

    Does Insurance Company Cover Services?

    *** The insurance companies do not typically cover marriage/couples counseling or sex therapy services. Majority of these concerns are not billable and are out of pocket costs. Family plans are necessary for the possibility to bill as a couple so individual policy plans aren’t accepted. **Consultation is required to determine if your problem or concern is a billable situation.***

    Please send the front and back of your insurance card as well as your ID to [email protected]. Our intake & Referral team will respond to you within 48-72 business hours with what your financial responsibility will be. This will include links to get on Chasity’s schedule as well.

    Good Faith Estimate

    Under Section 2799B-6 of the Public Health Service Act, health care providers and health care facilities are required to inform individuals who are not enrolled in a plan or coverage or a Federal health care program, or not seeking to file a claim with their plan or coverage both orally and in writing of their ability, upon request or at the time of scheduling health care items and services, to receive a “Good Faith Estimate” of expected charges.

    You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.

    Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.

    You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

    Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.

    If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

    Payment

    We accept debit cards, credit cards, FSAs and HSAs. All payments are due at the time of booking and card information is collected electronically via Square or Ivy Pay. This information is entered by the client.

    Cancellation Policy

    If you are unable to attend a session, please make sure you cancel at least 48 hours beforehand. Otherwise, you may be charged for the full rate of the session.

    All bookings are NON-REFUNDABLE so please contact the office if you need clarity. Clients who cancel will be issued a code to book future sessions.
